1. **Health Insurance:** I confirm that I have valid health insurance that covers any injuries or accidents that may occur during my participation in the event.

2. **Responsibility for Injuries:** I acknowledge and assume full responsibility for any injuries, accidents, or damages that may occur to myself or my property during the event.

3. **Medical Condition and Authorization:** I certify that I am in good physical health and able to participate in the activities. In the event of a medical emergency, I authorize the staff to obtain necessary medical treatment.

4. **Financial Responsibility and Insurance:** I understand that I am responsible for any costs associated with medical treatment and confirm that I have adequate insurance coverage for such expenses.

5. **Compliance with Camp Rules:** I agree to abide by all rules and instructions provided by the camp staff and understand that failure to comply may result in my removal from the event without refund.

6. **Assumption of Risk:** I acknowledge the inherent risks associated with camp activities and voluntarily assume all risks, known and unknown, arising from participation.

7. **Release, Indemnification, and Hold Harmless:** I hereby release, waive, discharge, indemnify, and hold harmless TruEdge Sports, its employees, agents, and affiliates from any liability, claims, or damages arising from my participation in the event.

8. **Cancellation and Refund Policy:** I acknowledge and agree to the camp's cancellation and refund policy as outlined in the official camp guidelines.

9. **Publicity:** I consent to the use of any photographs, videos, or other media taken during the event for promotional purposes by TruEdge Sports.

10. **Limitation of Liability:** I understand that TruEdge Sports shall not be liable for any direct, indirect, incidental, or consequential damages arising from my participation.

11. **Severability:** In the event that any provision of this waiver is deemed invalid or unenforceable, the remaining provisions shall remain in full force and effect.

12. **Governing Law and Jurisdiction:** This agreement shall be governed by the laws of Georgia and any disputes shall be resolved in the courts of Atlanta/Georgia
